Work report and print pdf

Hi folks,
I need to create an app to fill in the work report in pdf.
I want to create fillable text field in a certain position.
Then i want to print layout pdf.
I am attaching the pdf file to make you understand better.

Hi, This tutorial doesn’t work now if you upload the file in Thunkable you need to upload in a website. Because the Web viewer have a problem with locals file.
