This is not possible with Thunkable . I only know of a few apps available upon boot. One is a VPN I have used in the past.
I hope I am understanding your question correctly. Do you intend for the user to turn on their phone and the thunkable app be open already? If that is the case I would imagine this requires an extra level of permissions that are not currently available through the platform.

Iā€™m sorry! I just re-read your question. My above answer is absolutely not relevant.

The phones will not allow you to call from within the app. All Thunkable made apps will redirect the user to the native phone dialer for security reasons. I think most if not all apps do this.
